This is a complete nineteen-page medical billing and coding expert report, in the format we deliver. It covers four providers, including a hospital emergency department bill priced line by line against what other hospitals in the same market publish. The case in it is invented. The Medicare arithmetic is not: every one of those figures is read from the published CMS fee schedule file for that code, that locality and that date of service, and every one can be checked.

Five things most bill reports leave out.
Every charge line, priced. The date of service, the code, the amount charged, what was paid against it, and the published Medicare amount for that code in that locality on that date. One row per line of the bill.
The derivation behind every figure. A second exhibit showing which CMS release each benchmark came from, so your own coder can recompute any number in the report without calling us.
The hospital bill, priced against its own market. A hospital facility charge is not on the Medicare physician schedule, so it is measured against what comparable hospitals in the same market publish for the same service under the same revenue code. The sample shows an emergency department bill handled that way, line by line.
The coding findings. Edits billed together without a modifier, a new-patient visit billed twice inside the look-back, and services billed under codes the payer's own schedule does not accept.
Every limitation, with a number on it. Where a judgement call could have gone the other way, the report prices it both ways and says which reading it used. A limitation with a number attached can be weighed. One without a number cannot.
A coding opinion, not a medical one.
The report is signed by a Certified Professional Coder, so it addresses what was coded and what was charged. It does not address whether the treatment was necessary, whether the injury was caused by the incident, or the standard of care. Those need a physician. The sample states that boundary in its second section, which is where opposing counsel will look first.
